Data Protection

If you wish to obtain information that is held under the Data Protection Act, please complete the Data Protection Request Form and forward to the address below. If it is not possible to print this form, please request a form by writing to the address given below or by emailing DPA-FOI@GreenfieldsCH.org.uk. Please ensure that your full name and address is given in order that a form can be sent to you.   

Please note that there is a £10 administration charge for a Data Protection information request.   

Once the completed form and £10 charge has been received we have a maximum of 40 working days to investigate your request and then send you a response.

Freedom of Information

If you wish to make a Freedom of Information Request, please apply in writing stating the information required and for what reason it is required.  The Freedom of Information Publication Scheme provides details of the available documents and how to access them

Please either post your request to the address given below or email to DPA-FOI@GreenfieldsCH.org.uk. ; please ensure that your full name is given. 

Once your request is received, we have a maximum of 20 working days to investigate your request and send a response.
